Insurance Information

We make every effort to provide you with the finest care at an affordable cost. While we are considered an out-of-network provider to all insurance companies, we are able to file most major insurances. Please call the office to inquire if your insurance can be filed, as some require the office to be in network to file.  We encourage you to contact your insurance company in regards to coverage for your specific plan, as benefits vary depending on your policy.

We ask that you bring your insurance card/ information with you for your initial appointment to expedite determination of eligibility and benefits. Any changes in insurance coverage should be indicated to the office staff to avoid delays in filing claims.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment after that time frame.

Please remember you are fully responsible for all fees charged by this office, regardless of your insurance coverage.
